Login issues (Access Denied)

When I login, I initially see:

Once I click "Sign in", I see this:

When I click "Sign in with Salesforce", it takes me back to the first sign in prompt.

I tried clearing cache, different computers, different browsers, and in private/incognito but to no avail.

Please help, thanks 🙂
