I'm seeking a CISSP training program that is longer than the traditional week long boot camp. Preferably, an online, instructor-led program that spends at least one week on each of the domains. The only program I've found to date is offered by Cyber Vista, but I can't find any student reviews that rate the effectiveness of the program.
Couple questions:
1. Is anyone aware of CISSP training program that meets the above criteria?
2. Does anyone have first hand knowledge of the effectiveness of the Cyber Vista training program?

I do not have any experience with that program. I would like to share my experience and thoughts about bootcamps. In my experience bootcamps should be like the cherry on top of your ice cream, not the whole dessert. In other words, the bootcamp should be the climax of your studies, not the only thing you use to get trained. In my experiences, people who have used and expected a bootcamp to magically prepare them for the exam without any external or pre-studies, failed miserably.
Why can't you create your own bootcamp? Break it down into the domains and study each one for a month? I planned on a year for my CISSP exam journey and took (and passed) Network+ and Security+ before I went to the CISSP bootcamp 8 months later. Taking Network+ and Security+ was in effect like taking bootcamps for those domains.

I’ll second that.
If you don’t already have some feeder certifications, I recommend you start there. Things like CompTIA’s line will help you prepare and earn you certifications along the way. The Network+, Security+, CASP, and IT Project+ are all great places to gain knowledge that feeds the CISSP CBK and offer smaller chunks to bite off and grasp before you move to a broad topic qualification like the CISSP.

This course will allow you 120 days (from the date of purchase) to access the course material. In this format, you will be able to utilize the training material and break down each domain every week.

I'd suggest that you find other CISSP seekers in your area (have you got a local security group, perchance?) and form your own study group. Have each person take a (different) domain or topic, and prepare to give a presentation to, or lead a study by, the whole group. Meet weekly or monthly, to give yourselves time to study in between. It's much more effective than a bootcamp.
To get some resources, first off, read "Security Engineering," by Ross Anderson. (You don't even have to buy it, since he's put the old versions up online, for free.) For more specific topics, look into the references at http://victoria.tc.ca/int-grps/books/techrev/mnbksccd.htm (suggested references by domain), http://victoria.tc.ca/int-grps/books/techrev/mnbkscci.htm (reviews of the study guides), or http://victoria.tc.ca/int-grps/books/techrev/mnbksc.htm (a more complete list, but with some that aren't exactly recommended).
Unfortunately for some regions it's too expensive...
