Trouble with CC Application Form

Good morning from here.

I've been trying to enroll to the CC course now but I get stuck on the ISC2 Candidate Application Form, page 3.

Some check fields are grayed out, and the link to the Demographic Data Statement redirects me with an "about:blank#blocked" on brave (on firefox, "javascript:void(0);").

Am I doing something wrong?
